SSA Adult Disability Report
Allows you to file a disability report online with the Social Security Administration. The application will take approximately 90 minutes to complete and working Internet connection. It would also be a good idea to have a working printer as you will most likely want to print the report receipt for your records. The Online Adult Disability and Work History Report has seven sections in total: Overview, Identification, Medical, Work/Education, Remarks, Review, and Submit. Each section will provide detailed step b step guidance in relation to the application procedures.
Please note that the report does not have to complete all at once. You are allowed to save you progress and come back later. To do this please select the “Sign Off (finish later)” button at the bottom left of any page to stop working on the report, and then use the Reentry Number to come back to the section where you left off. If you have additional information you would like to relay please type it in the remarks section towards the end of the application. Once completed you will generate a copy of the entire disability report you submitted aka the receipt page. Please print this page for your records.
